A casa insieme

OBJECTIVE OF THE MEASURE
Pregnancy and the post-natal period involve needs that require timely, personalized support. The first 1,000 days of life are a crucial time for developing the skills needed for a child’s healthy physical, emotional, and cognitive growth.
Family Counselling Centers provide services to support motherhood and promote the wellbeing of the mother–baby–father unit.

New parents.

TYPE AND CHARACTERISTICS OF THE ASSISTANCE
The Continuity of Care Pathway "A casa Insieme", delivered by public and accredited private Family Counselling Centers in the ATS Val Padana area, aims to strengthen coordination between hospitals and community services by:

  • supporting new mothers immediately after birth and in the following weeks, with guidance on newborn care and breastfeeding;
  • providing professional support from a multidisciplinary team (midwife, gynecologist, social worker, health visitor, educator), either at the counselling center or at home;
  • promoting parenting support, including topics such as newborn sleep, preparing for a sibling, and age-appropriate play and reading, through one-to-one consultations and group sessions.

To better address needs at hospital discharge, midwives assign a color code (white, green, yellow, red) reflecting the current wellbeing of the mother and baby.
Once at home, new mothers may receive a call from their chosen Family Counselling Centre or contact the midwifery team directly for information and support. 

HOW TO USE THE ASSISTANCE
At the maternity units of the Cremona, Crema, and Mantova health authorities, staff will provide information about the program along with a form for new mothers to enroll.
